One question on the survey (#2) might cause a bit of confusion when answering it. This question is a ranking question (what is your first choice, second choice, etc.). In order for the survey to accept your answer, you may only pick one first choice, one second choice, etc. The survey will permit you to select more than one item however, the survey will not accept multiple choices. Hopefully the graphic below helps to explain this.
